B)   Pay in person (at North Shore Tennis Center at 501 72nd Street or Flamingo Park Tennis Center at 11th Street and Jefferson Ave, Miami Beach, FL)

C)   Send a check (payable to Greensquare Inc., P.O. Box 615, Miami Beach, FL 33119.)


            

Notification:
Upon completion of this application, you should receive a confirmation of your registration via e-mail in 1 to 3 business days.  It will give you a summary of your registration form and the total amount due.  Please note:  the completion of our registration form does not reserve you (or your child) a spot.  Payment must be received before registration is official.
